If you previously signed on as an AMC Consultant but stopped selling over the past few months or years, we encourage you to complete this form and become reinstated as an AMC Consultant. 

Once completed, an AMC Sales Manager will contact you to confirm we have received your form. They will also confirm the banking details that we have on record for you, or ask that you send new details through if required.
